Fundraising Opt Out

From time to time, UPMC mails fundraising materials to individuals throughout the region. If you receive these materials and wish to have your name removed from the mailing list, please submit a written letter to the appropriate group listed below. Your letter must contain all of the following information:

• Full name
• Complete address
• Phone number and area code
• Date of birth
• Date(s) of service
• Facility/practice where services were provided

Upon receipt of the written request that contains all of the above information, your record will be modified and every effort will be made not to mail fundraising information to you in the future.
