Medical Records
If you require a copy of your medical records, please be aware
that your medical records contain confidential information that we release only with your written consent. You may pick up a copy of the consent form in our office, or have us mail or fax you a copy. Please allow sufficient time to process your request.
Forms
If you need to have a physical examination form completed for
your child, and your child has been to the office for a regular
well-child exam within the last year, most forms can be filled out without another office visit. Your doctor will complete the form based on the previous examination. Please allow approximately
a week for us to complete the form.
