Making an Appointment
A physician's referral is needed to make an appointment at UPMC Pain Medicine. Have your PCP or referring physician fax or mail the required documentation to the UPMC Pain Medicine clinic location where you wish to make an appointment. Referring physicians should include the following information:
- prescription/letter for treatment
- patient demographics/insurance information
- radiology reports and office notes
