Staying-At-Home
The Staying-At-Home Program is a care coordination service providing peace of mind to seniors who live on their own, and their loved ones. It is a collaboration between UPMC Senior Communities and the University of Pittsburgh Institute on Aging, a respected authority on the issues faced by seniors and their loved ones.
The Staying-At-Home Program provides quality supportive care in the home and other residential facilities. Services include care management, medication management, and personal care. Our goal is to assist the client and his or her family in the performance of daily living activities, and to maintain the client’s independence and safety in the home or facility. The care plan is developed in partnership with the senior and family members so the client’s interests, abilities, likes, and dislikes are included.
