Venous Disease/Ulcers
Venous ulcers (also called non-healing wounds) can be a challenge to heal. The doctors of the UPMC Division of Vascular Surgery use the most up-to-date treatments to help heal venous ulcers. These treatments include evaluation of the veins to see if there is reflux or incompetent valves in the veins, the application of various types of dressings, and the use of substances called growth factors. Sometimes minor surgery (wound debridement) or vein ablation is required to remove any non-living tissue from a wound so that it can heal properly. The choice of treatment is tailored to the individual patient's needs. The following are descriptions of some of the available treatments.
Unna Boots
An Unna Boot is a moist gauze that is applied around the lower leg, from the region of the ulcer to just below the knee. This gauze then hardens to form a snug boot on the leg. The support from the boot helps improve blood flow in the veins and heal the ulcer. The boot stays on for 48 hours to two weeks and then is changed.
Transparent Dressings
A transparent dressing is a clear, plastic-like film that is applied over the wound. The dressing is changed about every 5-7 days. A support stocking is worn over the dressing to help hold it in place and improve circulation in the extremity.
Hydrocolloid Dressings
A hydrocolloid dressing is applied over the wound to promote healing. This dressing helps to remove any non-living tissue from the wound (debridment). It is changed about every 1-7 days, depending on how much drainage there is from the wound.
Growth Factors
Growth factors are investigational medications that are applied to the wound in the hope of promoting the growth of new tissue in the wound.
Compression
Compression of the leg is vital to healing venous wounds. The pressure from the compression dressing counteracts the pressure from the veins of the leg and allows the skin to heal. Most often this compression is through compression stockings which are prescribed by a physician or nurse and custom fit for the patient. In difficult cases Ace wraps can be used to provide the needed compression. Patient compliance with these measures is very important for healing.
