Contacting the Lung Transplantation Program
There are several ways to be referred to the Lung Transplantation Program at UPMC for a lung or heart-lung transplant evaluation. They include:
- referral by your primary care physician (PCP)
- self-referral
- friend or family referral
- prior transplant recipient

To refer a patient to UPMC's Cardiothoracic Transplantation Program or request a consult, please call UPMC’s 24-hour physician referral service at 1-800-544-2500 or 412-647-7000.
However you are referred, there are certain things you must do prior to your first visit.
First, you will speak with the transplant secretary or one of the coordinators in an initial phone evaluation.
He or she will request that you or your primary care physician mail all related information and medical records regarding your case to the office. Some of the information collected by phone includes (but is not limited to):
- personal information (age, address, marital status, etc.)
- original diagnosis, including prior history of medical care, and previous medical tests performed (such as catheterizations, PFTs, and echocardiograms)
- referring physician information
- insurance information
After the initial phone consultation, receipt of medical records, and a financial clearance, the initial evaluation will be scheduled. Since these first tests will be spread out over several days, you will need to make lodging arrangements. Prior to your appointment, you will receive a packet of information in the mail, which includes information on local accommodations, laboratory test requisition forms (you will have blood drawn locally, and the results will be sent to UPMC), a medical history questionnaire, and helpful information for you and your family.
Your initial evaluation will be scheduled after your phone consultation, and after we receive your medical records and financial clearance. We make every attempt to schedule the evaluation as quickly as possible to save you time and money and improve outcomes.
