Referral and Transplantation Process

At UPMC, we make the referral process easy. We'll help you manage your patient's expectations by explaining the liver transplantation process — from selecting candidates to providing post-transplant care.

How to Refer A Patient

To refer patients for liver transplant evaluations at UPMC’s Thomas E. Starzl Transplantation Institute:

  1. Review the transplant referral checklist.
  2. Provide all patient and referring physician information listed on the checklist.
  3. Mail, fax, or e-mail demographic and clinical summaries, along with patient records, to the UPMC liver transplant team at:

Thomas E. Starzl Transplantation Institute
UPMC Montefiore, 7 South
3459 Fifth Ave.
Pittsburgh, PA 15213

Fax: 412-647-5070
E-mail: transplant@upmc.edu

Or, call the 24-hour physician referral line at 1-800-544-2500.

If you have questions about the referral process, contact us any time.

What Happens Next?

  • Review and Evaluation
We review your patient's information to determine if a transplant evaluation is appropriate. If so, we schedule an evaluation.
  • The Transplant Process: Collaboration and Communication
The liver transplant team collaborates with referring physicians on pre-transplant medical management through post-transplant care. A member of the transplant team continually updates you on your patient’s progress throughout the entire transplant process.
