For Referring Physicians

At UPMC, we make the referral process easy. We'll help you manage your patient's expectations by explaining the lung transplantation process — from selecting candidates to providing post-transplant care.

To refer a patient to the Lung Transplantation Program:

To refer patients for lung transplant evaluations at UPMC’s Cardiothoracic Transplantation Program:

1. Review the transplant referral checklist. 
2. Provide all patient and referring physician information listed on the checklist.
3. Mail, fax, or e-mail demographic and clinical summaries, along with patient records, to the UPMC lung transplant team at:
