After Rehabilitation

There is a continuum of care available to patients with brain injuries throughout their recovery and rehabilitation. The patient care team and providers will decide the right time to come home from rehabilitation and possibly continue rehabilitation at an outpatient center.

Community re-entry

It is important to have a smooth transition from the hospital to the community. UPMC South Side offers community re-entry activities to help practice newly learned skills in “real life” situations. Examples of community re-entry activities can include going out for lunch or going shopping. The patient care team establishes goals for the outing. During the trip, problems can be identified; solutions can be addressed during therapy sessions.

Coming home

A patient often requires 24-hour supervision at home after discharge from the rehabilitation unit. The ability to provide such care for the patient may not be possible or within the family’s means. The case manager will work with the family to provide other solutions or suggestions.
