Referral for Services

Inpatient Rehabilitation

Most patients come to the IRR by referral from a physician or other member of the treatment team, most often after a stay in an acute care hospital. Patients may also come from other settings, including long term acute care or skilled nursing facilities.

The types of physicians who refer patients to the IRR are varied, but orthopaedics, internal medicine, neurology, and neurosurgery are some of the most common referral sources.

Referrals to any of the seven IRR facilities can be made by calling 1-877-AT-REHAB (287-3422).

Outpatient Rehabilitation

Many outpatient services are available through the Centers for Rehab Services. Referrals may not be necessary for all treatment. Please check with the facility when you make your appointment.
