Advance Care Planning

As a patient, you are in charge of deciding the type of medical care you want to receive — from undergoing a certain test to deciding if you want surgery to correct a medical problem. But when you are unable to make those decisions for yourself, an advance care plan directs your doctor, family and caregivers of your wishes for future medical care.

Having an advance care plan is important whatever your age or medical condition. Too often, we delay telling others what kind of care we want until an accident or sudden illness requires immediate action. An advance plan means your family won’t need to make difficult discussions about your care without your guidance. And, having time for advance discussion and reflection helps you and your family understand each other's views and values.
