ED U-Turn Program

Thousands of patients visit UPMC emergency departments (EDs) each year for treatment of injuries and ailments. We work hard to ensure that you receive appropriate care and preparation to safely return home and manage your recovery.

Our ED U-Turn program was developed as a way of bridging the transition of care after you leave the hospital. Designed jointly by the UPMC Visiting Nurse Association (VNA) and emergency medicine providers, the program provides patients with follow-up home visits from a nurse within 24 hours after discharge. Updates delivered to the ED doctor promote improvement in the patient’s condition.

The ED U-Turn program is designed to:

  • Increase medication and care plan compliance
  • Improve in-home safety for patients with a high risk for falls
  • Decrease the number of patients admitted to the hospital for observational care

Patients considered for the program include those receiving care for the following:

  • Asthma
  • Congestive heart failure (CHF)
  • Chronic obstructive pulmonary disease (COPD)
  • Diabetic care
  • Deep vein thrombosis (DVT)
  • Falls
  • Hypertension
  • Minor trauma
  • Pain
  • Wound care
  • Cellulitis

These home visits offer many benefits, including the opportunity for a nurse to provide educational guidance on these often-chronic conditions away from the chaotic hospital atmosphere. For older patients who experience a fall, a visiting nurse is trained to identify conditions that may contribute to an unsafe environment and make recommendations to prevent future falls.

The ED U-Turn program also offers patients an alternative to admission for observation following a medical event. Patients avoid the anxiety of a hospital admission with the benefit of a full medical assessment from a registered nurse within 24 hours.
