How to Request an Application

If you do not currently have privileges at a UPMC facility or you would like privileges at an additional UPMC facility, you will need to complete a UPMC CVO initial application packet. The packet consists of a Credentialing Information Form and associated documents.

Practitioners Employed by UPMC

If you are or will be employed by UPMC, contact the individual with whom you have been discussing other aspects of your employment.

Practitioners Not Employed by UPMC

If you are not nor will be employed by UPMC, contact the Medical Staff Office of the facility where you will be primarily providing services. To contact the Medical Staff Office, click here.
