UPMC St. Margaret Lawrenceville Family Health Center earns national recognition
PITTSBURGH, PA — UPMC St. Margaret is pleased to announce that the National Committee for Quality Assurance (NCQA) has recognized the UPMC St. Margaret Lawrenceville Family Health Center as a Physician Practice Connections® – Patient-Centered Medical Home™ (PPC®-PCMH™). The health center received Level 3 recognition — the highest level designated by the NCQA for using systematic, patient-centered, and coordinated care management processes.
The PPC-PCMH is a model of health care delivery that aims to improve the quality and efficiency of patient care. With collaboration of the American Academy of Family Physicians, the American Academy of Pediatrics, the American College of Physicians, and the American Osteopathic Association, the NCQA developed nine PPC standards to improve primary care and prevention.
The Lawrenceville Family Health Center, an academic teaching site for family medicine residents, is UPMC’s first family practice to earn Level 3 PPC-PCMH designation. Recognition was awarded for achieving the program’s key elements — access and communication, patient tracking and registry functions, care management, advance electronic communication, and performance reporting.
Level 3 recognition is valid for three years and allows the health center to be reimbursed for its quality care through insurances — the higher level of care, the higher level of reimbursement.
