Patient and Family Advisory Council Online Application

At UPMC, patients and their families are at the center of everything we do. To provide you with the platform to share your story and to help improve care for all patients, we have Patient and Family Advisory Councils (PFACs) at the following hospitals:

  • UPMC Passavant
  • UPMC St. Margaret
  • UPMC Presbyterian
  • UPMC Shadyside​

To join one of the councils, you or a loved one must have received care at the location where you would like to participate.

Please apply if:

  • You enjoy good discussion and like to ask questions.
  • You have the ability to share the points of view of patients and family members beyond your own experience.
  • You are a team player and like to take on new challenges.
  • You are able to make a time commitment to join us in person for meetings. 
You must be 18 years of age or older to apply.

Once you apply a representative from the council you chose will call you in 5 to 7 business days.
