UPMC Staying-At-Home Program
The UPMC Staying-At-Home program is a client-centered care coordination program offered to aging or chronically ill clients living in their own homes. The program uses a proactive, preventive approach to chronic care management, for optimal care for all clients, regardless of health or disability.

UPMC Living-at-Home Program
For those who meet specific income and clinical criteria, UPMC offers geriatric care coordination through a program called Living-At-Home, a community benefit program that helps clients age 70 and older who live in select communities to remain in their homes as long as possible.
