In some circumstances after a hospital stay, patients may need additional care at home as well as medical equipment while they recover from an illness or injury, or to manage an ongoing chronic condition.

Most often, home based care is provided by a visiting nurse agency. Visiting nurses offer many services to patients and families at home, including:

UPMC Staying-At-Home Program

The UPMC Staying-At-Home program is a client-centered care coordination program offered to aging or chronically ill clients living in their own homes. The program uses a proactive, preventive approach to chronic care management, for optimal care for all clients, regardless of health or disability.

UPMC Living-at-Home Program

For those who meet specific income and clinical criteria, UPMC offers geriatric care coordination through a program called Living-At-Home, a community benefit program that helps clients age 70 and older who live in select communities to remain in their homes as long as possible.
