Information for Patients Visiting HAPPI

What to Wear

You will need to change in to a patient gown for most exams at HAPPI

If you are scheduled for an EKG stress test, please wear active wear clothing and walking shoes.

What to Bring

Bring photo identification, typically your driver’s license or passport.

If your employer provided any forms prior to your appointment, please complete them before you arrive.

For travel health evaluations, bring your complete immunization record.

For commercial driver medical exams, bring medical documentation of health conditions such as:

  • Recent hemoglobin A1C
  • Results of EKG stress tests
  • Echocardiograms
  • Sleep apnea studies
  • CPAP treatment
