UPMC at Home provides health care services right in your very own home.

We offer convenient, compassionate, and complete care that meets your health needs — either after leaving the hospital or as part of maintaining your ongoing independence.

Our Home Health Services

Choosing the type of service that meets your needs may be complex.

UPMC at Home can help identify what you need and where you need it.

Many of our patients need physician-prescribed medications at home. Some have doctor's orders for home health nursing and rehab or medical equipment, such as oxygen, often following a hospital stay. Others with more complex needs may benefit from supportive services related to behavioral health or advanced illness.
