Contact UPMC Pain Medicine Program

A physician's referral is needed to make an appointment at UPMC Pain Medicine. Please use our Referral Form​ (PDF) to refer a patient to one of our sites or doctors. Have your PCP or referring physician fax or mail the required documentation to the UPMC Pain Medicine clinic location where you wish to make an appointment. Referring physicians should include the following information:

  • Prescription/letter for treatment
  • Patient demographics/insurance information
  • Radiology reports and office notes

If you would like to refer to a specific physician, please include his or her name on your referral form.
