At each of our skilled nursing and rehabilitation facilities we have a professional team including certified nurse practitioners, registered nurses, licensed practical nurses, and certified nursing assistants who work cooperatively to enhance quality of life and return residents to their highest level of independent functioning. Caring, dedicated staff provide comprehensive skilled nursing and rehabilitation services that address the physical, psychological, and social needs of each resident.

A registered nurse assesses the patient’s physical, psychological, and social needs to deliver individualized care while collaborating with the physician. The RN administers medications, performs skilled treatments, and supervises direct nursing care. The director of nursing (DON) and unit managers (UM) are registered nurses.
A licensed practical nurse provides patient care under the supervision of an RN, according to the individualized plan of care.
A certified nursing assistant works under the supervision of the RN and LPN, and provides or assists with basic care services.
Restorative nursing programs promote the patient’s ability to live as independently and safely as possible. Team members are responsible for coordinating these programs and work under the direction of the restorative nurse coordinator.
A certified registered nurse practitioner works with the medical director of the facility to provide on-site medical support for patients by monitoring both chronic and acute conditions.
Wound care nurses provide education and deliver specialized health care to individuals with wounds, ostomies, and incontinence.
Infection control nurses monitor active infections or potential risks for infections, and use appropriate isolation techniques to prevent transmission of infections while following Centers for Disease control guidelines.
A case manager is a member of the interdisciplinary team and works with the patient’s insurance company to provide the clinical information needed for skilled nursing reimbursement.
A social worker provides a wide variety of services, including assisting with psychosocial problems, counseling patients, advising family caregivers, planning for the patient’s discharge, and arranging for services needed at home.
Physical, occupational, and speech therapists provide therapeutic treatment modalities to promote independence and maximize the patient’s level of functioning.
The activities director is responsible for coordinating activities to promote physical and psychosocial aspects of care.
A registered dietitian oversees and provides recommendations for the nutritional care of patients.
