UPMC is committed to protecting the privacy of your medical information. To safeguard your privacy, you must complete and sign a form before records can be sent. On the form, you indicate what records are to be released and where they are to be sent.
Electronic Health Records
UPMC has now implemented an electronic health record at all of it's hospitals. View this document to find out if your health information is a candidate for electronic release on a CD. Electronic release is convenient and has a reduced cost. If you are interested, complete this form and forward to the appropriate hospital.
Paper Records
Pennsylvania has established the fees that hospitals and health care providers may charge for paper medical records. This chart gives a general idea of how much you will be charged. When you send your request for records, you will receive an invoice that tells the exact charge.
