  • Care Management in VA Home Based Primary Care (HBPC)
    Provides HBPC nurses with a basic understanding of their role in the care management process, including discussion of quality improvement strategies and available resources. Presented by an Advanced Practice Adult & Gerontology Clinical Nurse Specialist with more than 25 years of expertise in providing medical care management services to adults and elders.
  • Managing Polypharmacy in Home Based Primary Care (HBPC) Nursing: What does the count have to do with it?
    A university nursing instructor presents an overview of polypharmacy for HBPC nurses and other healthcare professionals. The presentation explains potential negative impacts that polypharmacy can have on older adults, and provides guidance for identifying interventions that can be used in homecare clinicians’ daily practice to help prevent or mitigate negative outcomes related to polypharmacy.
  • Management of Behavioral Problems in Dementia by the Home Based Primary Care (HBPC) Team
    A geropsychiatrist describes non-pharmacological interventions for managing common behavioral symptoms among patients with dementia. The practical strategies can be implemented immediately by HBPC nurses, other staff, and anyone who interacts with people with dementia.
