This staff enhances members’ ability to manage their medical conditions and improves their quality of life by facilitating access to community resources for transportation, housing, and food security. Community Care serves more than one million people on medical assistance in 41 counties, and has been a resource to count on for more than 20 years.

As one of three organizations selected by the Commonwealth of Pennsylvania to coordinate medical care, long-term services, and support for individuals who qualify for both Medical Assistance and Medicare, UPMC is bridging a gap between housing and health care services.
