To Arrange a Patient Transfer to UPMC

When you call to request a transfer, please provide the following information:

• Patient’s name and DOB.
• Referring attending’s name and contact number.
• Current patient location and unit phone number.
• Diagnosis and medical service(s) needed.
• Special needs (isolation, ventilator, IVs, etc.).
• Most recent vitals.
• Use this for more details when initiating a transfer with MedCall Western PA.
