The licensed clinical social worker works with your primary care doctor to provide short-term assessments and behavioral health counseling.

We can help with concerns such as:

  • Suicidal thoughts
  • Self-image
  • Stress
  • Addiction
  • Relational, parenting, and aging problems

Our behavioral health specialist has expert training in:

  • Psychotherapy
  • Assessment and diagnosis
  • Substance abuse treatment
  • Crisis management

If you need longer term care or complex treatment, we can connect you with the right resources and providers.

Treatment may require a referral from your PCP.

For people living with diabetes, our experts can help you better understand and manage your disease.

Our experts have training in prediabetes and diabetes prevention, and will help with:

Medical Social Worker (MSW)

Our MSW can:

  • Assess social, emotional, environmental, financial, and support needs.
  • Work with you, your PCP, and family to help arrange care among other services.
  • Help those who may be living with a chronic degenerative illness, terminal illness, or mental illness.
  • Assist with other social, financial, emotional, or housing issues.
  • Help patients after discharge from a hospital or with the transition to another level of care.

Monday through Friday: 7:30 a.m. to 7:30 p.m.;
Weekends: 9 a.m. to 5 p.m.

We provide convenient access to primary care for people with a busy lifestyle.

Some of our walk-in services include:

  • Acute care and treatment for minor conditions
  • Physicals
  • Vaccinations

If your health insurance plan requires a copay, the cost for the Walk-in Center visit may be at the primary care copay.​​​​
