Post-Acute and Long-Term Care at UPMC in Central Pa.

After a hospital stay for injury or illness, some patients may need continued medical care at home or at a specialized medical facility such as a rehabilitation hospital or a skilled nursing facility. This is also called post-acute care.

UPMC in Central Pa. offers post-acute and long-term care. We have a team of physicians and nurse practitioners that provide specialized medical care for patients who transfer from one care center to another, even if they require long-term care in a skilled nursing facility. Our providers are specialized in internal medicine, family medicine, geriatrics, and hospice and palliative medicine. Their goal is to provide quality care specific to your health care needs and long-term goals.

What are the benefits of the Skilled Nursing Facility Post-acute and Long-term Care Program?

  • Provides a smooth transition and good communication from hospital to care facilities.
  • Prevents hospital re-admissions and shortens time in care facilities.
  • Provides oversight of medication safety and necessity.

How do I participate in the Skilled Nursing Facility Post-acute and Long-term Care Program?
