If you choose one of the skilled nursing facilities listed below, the facility's staff will coordinate your care and contact us directly if they assign one of our post-acute care providers to you.

Members of your UPMC post-acute care team will oversee your medical needs at the skilled nursing facility and maintain communication with your physician, as well as assist you with planning any medical care you may need after you leave the skilled nursing facility.

UPMC Post Acute Care Network (PACN)

Skilled nursing facilities in Dauphin, Cumberland, Perry, Lancaster, Lebanon, and York counties have partnered with UPMC in Central Pa. to form the PACN. This network includes an interdisciplinary team of UPMC physicians, nurse practitioners, and care coordinators that work collaboratively with skilled nursing facilities to improve the transition of care and quality outcomes for our senior population. Our goal is to provide access to the best care, at the right place, at the right time. The PACN provides clinical services, geriatric education, and evidenced-based treatment protocols so facilities can care for the senior population.
