- Make recommendations to your health care providers
Registered Dietitian
A registered dietitian (RD) is a food and nutrition expert who helps pre-diabetic individuals and other patients with nutrition concerns. Your RD translates the science of nutrition into practical solutions for a healthier lifestyle.
Wellness Guide
Wellness guides are extensively trained medical assistants who teach self-management support to patients with new or uncontrolled chronic disease.
Your wellness guide can help you achieve health care goals recommended by your doctor, such as:
- Quit smoking
- Increase your physical activity
- Lose weight
- Improve your nutrition
Medical Social Worker
Your medical social worker can help assess your social, emotional, environmental, financial, and support needs.
He or she will also work with you, your family, and your doctor to create a coordinated plan of care, including helping patients with:

- Financial and housing issues
- Discharge from hospital

Behavioral Health Counselor
Your behavioral health counselor, or licensed clinical social worker, is highly trained in psychotherapy, assessment, diagnosis, substance abuse treatment, and crisis management.
