Our new team-based approach helps you with your health and wellness, as well chronic conditions. Your primary care doctor coordinates your care — and our Expanded Care Team provides specialized support tailored to your physical and mental health needs.

Certified Diabetes Educator®

A certified diabetes educator (CDE) helps patients living with diabetes to better understand and manage their condition.

Your CDE is trained in prediabetes and diabetes prevention, and will help you:

  • Manage and adjust your medications
  • Change your diet and develop a plan
  • Make recommendations to your health care providers

Registered Dietitian

A registered dietitian (RD) is a food and nutrition expert who helps pre-diabetic individuals and other patients with nutrition concerns. Your RD translates the science of nutrition into practical solutions for a healthier lifestyle.

Wellness Guide

Wellness guides are extensively trained medical assistants who teach self-management support to patients with new or uncontrolled chronic disease.

Your wellness guide can help you achieve health care goals recommended by your doctor, such as:

  • Quit smoking
  • Increase your physical activity
  • Lose weight
  • Improve your nutrition

Medical Social Worker

Your medical social worker can help assess your social, emotional, environmental, financial, and support needs.

He or she will also work with you, your family, and your doctor to create a coordinated plan of care, including helping patients with:

  • Chronic degenerative or terminal illness
  • Mental illness and other social or emotional issues
  • Financial and housing issues
  • Discharge from hospital
  • Transitioning to a different level of care

Behavioral Health Counselor

Your behavioral health counselor, or licensed clinical social worker, is highly trained in psychotherapy, assessment, diagnosis, substance abuse treatment, and crisis management.

He or she will provide you with short-term assessment and counseling and will help patients with concerns such as:

  • Anger management
  • Depression
  • Suicidal thoughts
  • Self-image
  • Stress
  • Addiction
  • Relational, parenting, and aging problems
