Virtual Diabetes Care is a 12-week program to help people with A1C levels greater than 8 gain control of their diabetes.
Through this program, you use innovative software to track your health.
Who Should Sign Up for Virtual Diabetes Care?
This program is for you if you have:
- A1C levels of 8 or higher and want to improve.
- Access to a smartphone or computer and the internet.
Your doctor will discuss the program with you, including the Tidepool software that you need to download data.
How Does Virtual Diabetes Care Work?
Using the tools of telemedicine, you can take part online from the comfort of your home via your smartphone or computer.
During this 12-week program, you will have weekly virtual video visits with our diabetes team to review your:
- Blood sugar data.
- Diet and exercise.
- Any medicines you're taking.
The software shows you results from your:
- Insulin pump.
- Continuous glucose monitor (CGM).
- Blood glucose meter.
You can also track meals, exercise, and daily events and see tips on how to improve your health.
What to Expect During Your First Diabetes Video Visit
You'll have your first virtual visit with a diabetes educator to explain the goals of the program.
This will last around 30 to 60 minutes.
How Can I Sign Up?
Ask your diabetes provider or educator if the Virtual Diabetes Care program is right for you.
