New Patients

Your first visit to a primary care practice is very important.

Before your visit, you will need to fill out the medical history form. This will help your new primary care provider coordinate your individual health care needs.

During Your Appointment

During your primary care visit, participate in your health care. Don't be shy about asking questions and taking notes.

Taking notes during your visit can help you remember the specific information you and your doctor discussed.

Prescription Refills

Once you have established a relationship with your primary care provider, you can request prescriptions refills during your office visits, by telephone, or online through your MyUPMC account. If you call the office, there is a 48 to 72 hour turnaround time to process the refill.

If you are requesting a refill, you will need to provide the following information:

  • Your name
  • Phone number where you can be reached
  • Birth date
  • Name of drug to be refilled
  • Dosage or strength of the drug
  • How often you take the drug
  • Phone number for the pharmacy where you prefer to pick up your prescription.

Not all requests for refills will be approved. There are some medications that may require an appointment with a provider before the refill will be granted, even if you have been taking the medication consistently. If you have not seen your doctor for several months, we will ask you to make an appointment before refills can be authorized.

Prescription refills are granted at the discretion of the health care provider with consideration given to the needs and health of the patient, appropriate timing of the refill, the effects of the requested medication on the patient, and the patient’s progress while taking the medication.
