Successful discharge planning from an acute hospital stay leads to reduced readmissions (returns to the hospital for care) and better outcomes.

Discharge planning begins as soon as our staff admits you. This way, you can have the best discharge plan for you.

The Clinical Care Coordination and Discharge Planning team meets with you as soon as you arrive. This combined team figures out what current care you have in place and what care you'll need when you leave. Their recommendations will support your path to wellness, making the need to return less likely.

The Centers for Medicare and Medicaid Services (CMS) monitors our work to improve health care for all people. According to CMS, "Hospitals are encouraged to improve communication and care coordination to better engage patients and caregivers in discharge plans and, in turn, reduce avoidable readmissions."
