Patients can be considered homebound for multiple reasons, including chronic illness, a condition where leaving the home is contraindicated, need for assistive devices for mobility, use of specialized transportation or requiring physical assistance to leave the home. Being homebound creates a huge barrier to maintaining and improving the patient’s health. These patient’s generally lack primary care due to being unable to physically travel to the doctor’s office, unless patient is taken to the hospital with an emergency. This creates fluctuations in patient status, increasing hospital admissions.
Our Home Visit Program is an innovative, comprehensive, patient centered approach, responding to the medical needs of home-bound patients, while addressing the social determinants of health. Through regularly scheduled visits, we bring primary care to the homebound patient. These patients are able to keep up with their wellness visits, follow up appointments, blood work, and vaccinations from their home. When basic primary care is maintained and the patient is monitored by professionals, patients can more effectively manage their chronic illness and overall health, stabilizing their conditions and decreasing hospital admissions.
Our team becomes familiar with patients, caregivers, and family members, forming strong relationships that promote a better quality of life for the homebound, chronically ill patients, and those who have significant barriers to obtaining consistent healthcare service.
