Diabetes Programs at UPMC Western Maryland

Individuals with the following characteristics are at increased risk for developing prediabetes and type 2 diabetes:

  • 35 years of age or older
  • Overweight
  • Family history of diabetes
  • Physically active fewer than three times per week
  • Had diabetes while pregnant (gestational) or gave birth to a baby that weighed more than 9lbs

If You’ve Been Diagnosed with Type 1 or Type 2 Diabetes

Living Well with Diabetes is a diabetes self-management program with interactive group education. The one-day (6 hours) program, including a 2-hour follow-up, includes a healthy lunch.

Living Well with Diabetes:

  • Requires a physician referral
  • Covered by most insurance plans
  • Approved by the American Diabetes Association

The teaching team includes a certified diabetes educator, nurse, registered dietitians, pharmacists, and a wellness coach/activity instructor.

A family member or support person is welcome to attend with you.

If You’ve Been Diagnosed with Prediabetes

Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Prediabetes puts you at increased risk of developing heart disease and stroke as well as type 2 diabetes.

You can have prediabetes for years but have no clear symptoms. You can get a simple blood sugar test to find out if you have prediabetes. Ask your doctor if you should be tested.

If you have prediabetes, losing a small amount of weight if you’re overweight and getting regular physical activity can lower your risk of developing type 2 diabetes.

  • Health Coaching – Free one-on-one personalized health and wellness planning for individuals who want to make long-term behavior changes to improve overall well-being and health.
  • Medical Nutrition Therapy – A registered dietitian nutritionist can provide practical and easy-to-follow advice personalized for your needs or recommend free programs to help.
