Individuals with the following characteristics are at increased risk for developing prediabetes and type 2 diabetes:
- 35 years of age or older
- Overweight
- Family history of diabetes
- Physically active fewer than three times per week
- Had diabetes while pregnant (gestational) or gave birth to a baby that weighed more than 9lbs

If You’ve Been Diagnosed with Type 1 or Type 2 Diabetes
Living Well with Diabetes is a diabetes self-management program with interactive group education. The one-day (6 hours) program, including a 2-hour follow-up, includes a healthy lunch.
Living Well with Diabetes:
- Requires a physician referral
- Covered by most insurance plans
- Approved by the American Diabetes Association
The teaching team includes a certified diabetes educator, nurse, registered dietitians, pharmacists, and a wellness coach/activity instructor.
A family member or support person is welcome to attend with you.
If You’ve Been Diagnosed with Prediabetes
Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Prediabetes puts you at increased risk of developing heart disease and stroke as well as type 2 diabetes.
You can have prediabetes for years but have no clear symptoms. You can get a simple blood sugar test to find out if you have prediabetes. Ask your doctor if you should be tested.
If you have prediabetes, losing a small amount of weight if you’re overweight and getting regular physical activity can lower your risk of developing type 2 diabetes.

- Medical Nutrition Therapy – A registered dietitian nutritionist can provide practical and easy-to-follow advice personalized for your needs or recommend free programs to help.
