The UPMC Heart Failure Center provides tools to help you manage your symptoms at home, reduce your hospital admissions and improve your quality of life. Through regularly scheduled Center visits, we will monitor your heart and lungs to determine the stage and progression of your illness.

Our Center is managed by nurses and supervised by Brian Corbally, DO, advanced heart failure cardiologist and medical director. Our nursing team is experienced in caring for heart failure patients and certified in cardiovascular care.

In recognition of the quality care we provide, our Heart Failure Center has been awarded Gold Plus status by the American Heart Association's Get with the Guidelines® program.

Our Services

Following a hospitalization for heart failure, our Certified Registered Nurse Practitioners will work closely with you to plan your ongoing heart failure care. If you are a patient of the UPMC Heart and Vascular Institute , you have the option to receive ongoing heart failure care through our Heart Failure Center.

If you are admitted to the hospital, a nurse navigator from the Heart Failure Center will visit you to follow your progress, help determine why you were hospitalized and assist the health care team in developing a plan for your future care. This plan may include visits to our Heart Failure Center to help you learn how to manage your heart failure.

If you are discharged from the hospital to home care, a rehabilitation facility or a skilled nursing facility, our nurse navigators will contact these agencies to coordinate your heart failure care. In addition to working with your other health care providers, our Heart Failure Center team provides a full range of services, including:

  • Education on a variety of topics for living with heart failure.
  • Medication management.
  • Referrals to other UPMC services in Central Pa.
  • Telemanagement to help you manage your heart failure symptoms at home.
  • Extended visits, when needed.
  • Consults with dietitians, pharmacists, smoking cessation and diabetic educators, if needed.
