Release of Patient Information is located on the first floor of UPMC Somerset, near the main entrance.
To get to the department, enter the hospital at the main entrance. Walk past the registration desk and down the hall. Once you pass the silver elevators and can’t go any further, make a right and the office will be on the left side of the hallway. The department will be providing services between the hours of 9 a.m. to 4 p.m., Monday through Friday. Patients may park in the parking lot at the main entrance. The telephone number for obtaining patient records is 814-443-5158.
An authorization form for the release of medical information must be completed for all requests. Download a copy of the release of information form (PDF). Patients may also request records via the internet by contacting SOM-records@upmc.edu.

Please note: If you still need to log in to your current portal, you will have view-only access for 180 days following Sept. 27,2025. During the view-only period, you will be able to log in and view or download existing medical records. You will no longer be able to schedule appointments or message providers in your current portal.
