In order to see that everything is in place when you are ready for discharge from the hospital, we begin planning on the day of your admission.
Our data shows that getting discharged earlier in the day decreases your chances of being readmitted to the hospital. It is our goal to have you discharged as close to 11 a.m. as possible on your day of discharge. Being prepared for discharge and your care at home will aid in your recovery and get you back to your normal activities.
We want to understand what your needs, goals and preferences are for after care. How do we prepare you for discharge?
If you need assistance planning your care after discharge, please call our Social Work and Discharge Planning Department at 717-782-5574 between 8 a.m. to 4:30 p.m.
