Discharge - Preparing

In order to see that everything is in place when you are ready for discharge from the hospital, we begin planning on the day of your admission.

Our data shows that getting discharged earlier in the day decreases your chances of being readmitted to the hospital. It is our goal to have you discharged as close to 11 a.m. as possible on your day of discharge. Being prepared for discharge and your care at home will aid in your recovery and get you back to your normal activities.

Preparing for Discharge

We want to understand what your needs, goals and preferences are for after care. How do we prepare you for discharge?

  • A Case Manager or Social Worker on our team will work with you to develop the best plan and discharge options for your personal circumstances and condition.
  • At the time of discharge, your nurse will give you discharge papers that list your medications, follow-up appointments and other instructions about your after care.
  • We will also give detailed information about your stay to your family doctor and help you schedule an appointment.
    • A follow-up appointment with your family doctor after you leave the hospital is very important. If you do not have a primary care physician or family doctor, we can assist you in finding one for follow-up care.
