​Blended Service Coordination

The Blended Service Coordination (BSC) program at UPMC Western Psychiatric Hospital provides services to individuals with mental health conditions, including children and adolescents, who meet eligibility requirements.

BSC provides assistance in gaining access to many needed services, including:

  • Medical services
  • Social services
  • Educational services

In addition, the BSC program provides assistance with:

  • Linking individuals and families with needed services
  • Monitoring of service delivery
  • Gaining access to needed services
  • Assessments and service planning
  • Problem resolution
  • Informal support network building
  • The use of community resources

Seven adult BSC teams, two child/adolescent teams, and two specialized teams provide services for homeless individuals through homeless outreach and Housing and Urban Development (HUD) housing assistance.

Our two-person, enhanced intensive service coordination team, delivers specialized services to a population of individuals with mental health conditions who are identified and referred by Community Care Behavior Health.

This is a time-limited (90-day) extended engagement program that attempts to engage very challenging, high utilizers of emergency department and inpatient hospitalizations and link them to services and to assist them in accessing those services.
